About Course

Fixing only the obvious cause of an accident means it will probably happen again. Based on HSE’s guidance HSG245, this course explains immediate, underlying and root causes, how to analyse an incident by repeatedly asking why, how to look beyond ‘human error’, and how to turn findings into a SMART action plan.

This course is based on HSE guidance and gives general information. It is not legal advice. Follow your organisation’s investigation procedure and RIDDOR reporting requirements.
